• Shift: 8:00am - 4:30pm (8 Hour Days)\n

• Estimated Weekly Pay: $1,930 - $2,144\n
• Assignment Duration: Minimum 13 weeks, with potential extension\n

\n\nSocial Worker Qualifications\n

• Active Illinois Social Work License (LCSW or LSW required)\n
• Minimum 1 year of clinical social work experience in a healthcare setting\n
• Experience with ventricular assist device (VAD) care preferred\n
• Experience supporting heart failure patients preferred\n
• Strong communication, organizational, and case management skills\n

• Deliver social work services to heart failure and VAD patients and their families\n
• Perform psychosocial assessments and develop individualized care plans\n
• Collaborate with interdisciplinary healthcare teams for patient care and discharge planning\n
